Healthcare services including doctors & hospitals offering online consultations, booking appointments, and receiving health updates via mobile platforms, having data access is no longer a luxury, but a necessity. It enables individuals to track their health, manage prescriptions, and keep up with appointments, making healthcare more accessible and efficient.

Is also a key tool for managing our money. With online banking, financial planning tools, and access to real-time updates, individuals can manage their finances securely from anywhere, anytime. Eligibility

• 18+ years old AND from a low income household
• Only one application per individual is allowed, you can refer someone else in need of support.
• AND/OR have no access or insufficient access to the internet at home
• AND/OR have no or insufficient access to the internet when away from the home
• AND/OR cannot afford an existing monthly contract or top up
• Subject to availability and your needs, you may receive a SIM card from O2, Vodafone or Three.

Data Sharing

• Your application details will be shared with the Good Things Foundation, which administers the SIM card program, to facilitate your participation in the initiative.
• By submitting the application form, you consent to Derbyshire Community Bank collecting and processing your personal information as outlined in these terms. All information provided must be accurate and complete at the time of application.
• Your information may also be shared with other third-party partners involved in the administration or delivery of the program.
• Derbyshire Community Bank and its partners will process your data in compliance with the General Data Protection Regulation (GDPR).

SIM Card Usage

• The free SIM card is intended for personal use only and must not be resold or used for commercial purposes.

Delivery

• SIM cards will be sent by post to the address provided on your application form.
• Please note that during the Christmas season, delivery times may be delayed due to high postal service demand. While Derbyshire Community Bank will process applications promptly, delivery within a specific timeframe cannot be guaranteed.
• Derbyshire Community Bank is not responsible for SIM cards lost or delayed in the post.

Availability

• SIM cards are available on a first-come, first-served basis and are subject to availability.
